Please arrive at least 15 minutes early for your appointment to facilitate registration. Also, please bring all your medications in their original prescription container.
Every effort is made to keep scheduled appointment times. Occasionally, however, a hospital emergency might cause a delay. If this happens, we will inform you and offer you an alternate appointment if necessary.
As a courtesy to your fellow patients, you must have an appointment to see your physician. If you are experiencing symptoms, please call the office and ask to speak with an office representative. As part of our total commitment to your health care, our physicians work on a rotating after-hours, weekend and holiday schedule. In case of an after-hours medical problem, you may call the office 24/7 and the on-call physician will address your needs. Missed appointments are one of the biggest contributors to the inefficient operation of any medical office. Patients who miss their appointments unproductively consume the scarce resource of the physician's time while other patients must wait for appointments. In respect for your fellow patients, please notify the office at least 24 hours in advance if you will not be able to keep your appointment. Failure to keep an appointment without 24-hour notice may result in a service charge.
We participate in a variety of plans including Medicare and AHCCCS. Please call us with any questions regarding the insurance plans we accept or other billing concerns. We do our best to keep abreast of what the different plans cover, nevertheless, it is ultimately your responsibility to know what your insurance plan covers. If your medical insurance company does not pay the full balance or denies the claim, any unpaid balance is due within 30 days following the appointment
